Refund Policy
If TennCare rejects your QIT because of a defect in the QIT form itself, we will refund your purchase price in full to the original payment method.
To request a refund, you must provide a copy of the written denial letter from TennCare specifically stating that the form itself was defective, and proof that the form was not altered, modified, or completed incorrectly.
Refunds are not available if:
The denial is based on how the form was completed or signed, or the form was completed incorrectly or with inaccurate information
The form was not signed, notarized, or executed as required
The denial relates to how the bank account was created or funded
TennCare issued a denial for reasons unrelated to the document template itself (for example, missing information, improper use, or other eligibility issues)
